MENTAL HEALTH PROFESSIONAL MEMBERSHIP INFORMATION

As a mental health professional, you also have the option of becoming an MHA member with the benefit of having your name, address and phone number listed under a choice of categories in our Resource section.  (This does not include a company/organization listing.)

However, you must meet the following conditions and submit the required documentation, for a listing in the Resource section of our website:

• You must be licensed in NYS and in practice a minimum of two years
• You must provide MHA with a copy of your NYS Certificate of licensure

To become a member as a mental health professional with a listing on the MHA website, download and complete the "Mental Health Professional" Membership Application, provide the relevant documentation and mail to Mental Health Association, 199 N. Wellwood Avenue, Lindenhurst, NY 11757, or you may fax it to the MHA at 631-225-1708.  In addition, please include documentation of any accredited post-graduate training classes or specializations you have completed.
